A 5-day training course providing both a theoretical and practical overview of the Tripod Beta methodology, enabling delegates to support and conduct incident investigations and analyses

About Tripod Beta:

Tripod is a theory for understanding incidents and accidents, based on the Swiss Cheese model. It was developed to explain how and why incidents happen and allow the root organisational causes and deficiencies to be uncovered and addressed. This 5-day training course will provide a theoretical overview of the Tripod Beta methodology and teach delegates how to apply this in practice, helping delegates uncover not just how an incident happened but also why it happened: the management controls that should have prevented it and the systemic organisational issues that can lead to future incidents.

Why the Tripod Beta training course?

  • The course is accredited by the Stichting Tripod Foundation, the custodians of the Tripod Beta methodology
  • Delegates will receive a free copy of the official Tripod Beta user guide, ‘Tripod Beta: Guidance on using Tripod Beta in the investigation and analysis of incidents, accidents and business losses’, worth £150
  • Successful delegates will be awarded Bronze Practitioners certification by the Stichting Tripod Foundation. Bronze Practitioners are recognised as being able to begin to use Tripod Beta and will be invited to undertake further accreditation to become a Gold Tripod Beta Practitioner.

Learning outcomes:

Upon completing this training course, delegates should be able to:

  • Use the Tripod Beta analysis methodology for analysis of simple incidents.
  • Support their organisation in use and implementation of Tripod Beta in incident analysis.
  • Attempt at the knowledge assessment for Bronze accreditation as Tripod Beta Practitioner.

Course overview:

Each training day is organised as 2 hours interactive online session with the trainer and 2 hours of self-managed group work with cases, which is shared with the trainer after completion – all on the same day.
Pre-work: Reading of a selection of the Tripod Beta user guide and doing small individual case-work which is shared with the trainer minimum 3 days before the course.

The topics covered during the course are:

Day 1:

  • Introduction to incident analysis in general.
  • Tripod Beta and other methodologies.
  • Tripod Beta: The elements and the terminology.Making the core diagram
  • Introduction to barriers and barrier assessment.
  • Feedback on homework.
  • Introduction to new homework (cases).

Day 2:

  • Re-cap of day 1.
  • Introduction to barrier analysis.
  • Feedback on homework.
  • Introduction to new homework (cases).

Day 3:

  • Dive into barrier analysis.
  • Identification of immediate cause, preconditions and underlying causes.
  • Introduction of Basic Risk Factors.
  • Feedback on homework.
  • Introduction to new homework.

Day 4:

  • Re-cap of all Tripod Beta elements.
  • Feedback on homework.
  • Introduction to new homework.

Day 5:

  • Re-cap of everything – in preparation for the knowledge assessment.
  • Individual timings for the knowledge assessment for Bronze accreditation as Tripod Beta Practitioner.


Assessment:

The EI Tripod Beta training course will be assessed by a 45 minute multiple-choice examination. Those who pass this knowledge assessment will be awarded Bronze Practitioner certification by the Stichting Tripod Foundation.

Qualification:

On successful completion of the Tripod Beta exam, delegates will achieve a Bronze Practitioner certificate, demonstrating their understanding of the Tripod Beta methodology. Bronze Practitioners will exhibit an excellent theoretical understanding and can begin assisting with low-level investigations.

Upon attaining Bronze Practitioner status, delegates will be invited to continue towards becoming an accredited Gold Tripod Practitioner and conduct more high-level and complex incident investigation and analyses.

Lisbeth Holberg is an independent consultant since 2009, with a core business around making safety tangible to managers and employees, also when it comes to learning from incidents. She is a leading expert in Tripod Beta and how to apply incident investigation across all industries. She has a solid client base in offshore wind (construction, operations and maintenance), oil & gas, thermal power and construction industry. She is an accredited trainer since 2013 and has delivered many Tripod Beta trainings as in-house training for her clients and as open trainings for participants from a mix of companies. Lisbeth has been the Lead Investigator in more than 35 investigations of highly serious incidents. Before settling as an independent consultant, Lisbeth worked in oil & gas, with all aspects of environmental, health, safety and quality aspects involved in exploration and production. Lisbeth has worked with Tripod Beta since 2006. Lisbeth is known as an engaging and inspiring trainer, who creates highly interactive sessions which easily transfers to the student’s daily work.
