3445 Guidance for integrity root-cause analysis

This publication helps the integrity community to learn from low criticality incidents.

The guidance introduces the topic of root-cause analyses (RCA) and how to address and capture lessons from 'more frequent - low consequence' incidents within the integrity management system. Along with its companion Root-cause analyses (spreadsheet) Tool, this guidance provides a means of addressing the same type of level of analysis for low criticality integrity-related incidents (materials and corrosion) as is more commonplace on high consequence, high criticality failures and near-misses.
